Happy Birthday from GBU Life!

As a gift from GBU Life to you, we would like to extend the opportunity to participate in a "Birthday Match". Make a donation to your favorite 501(c)3 organization on your birthday and we will match your donation up to $100! After reviewing the donation options available and making your donation, complete the application below.

Facebook Birthday Fundraiser
Start a birthday fundraiser on Facebook. Take a screenshot of the results of your fundraiser (including how much money was raised and the date of the fundraiser) and upload it during the application process. For this, we will match up to $250!

Make an Online Donation
Not a fan of Facebook? No problem. If your selected organization takes online donations on their website, you can make a donation there and upload your receipt during the application process. If the organization does not issue receipts, take a screenshot of the confirmation of the donation.

Make a Donation via Check
You can also just write out a check for a donation and mail it to us. Be sure to write the check out to the name of the organization. We will send your check and ours to the organization.
